We appreciate the opportunity to serve our patients and provide the highest standards of dental care.
Please call our office (317) 815-5552 to schedule a consultation or a treatment appointment, or visit the contact us page to email our office.
Please prepare for your visit by providing the following information at the time of your 1st appointment:
- 1. State issued photo ID.
- 2. Your referral slip and any x-ray films if applicable. Digital radiographs can be emailed. Please talk to our staff and we will request them from your dentist.
- 3. Please complete Secure Patient Forms.
- 4. Please inform us prior to your appointment of any allergies, medical conditions, and medications. Your physician's name and phone number may be needed. If you have been advised to take an antibiotic prior to the dental procedures for the heart conditions, prosthetic joints, or other medical conditions, please make sure to follow doctor's recommendations.
- 5. If you have dental insurance, please inform us, so we will give you an estimate of your insurance benefits for our procedures. Please bring identification card with you. This will allow us to help you process insurance claims.
- 6. Our mission is to provide high standards of dental care in a relaxed environment. Local anesthesia (numbing) is used for all the patients to have procedures done painlessly. While this is sufficient for most of our patients to feel comfortable, some patients benefit from various forms of sedation. We are offering Nitrous Oxide (laughing gas) and oral conscious sedation to make patient's experience as relaxing as possible. If you feel that you need to have sedation, please talk to our scheduling staff for instructions.
- 7. An endodontic consultation will be completed prior to the initiation of any treatment to confirm the need in treatment. If your doctor or you have questions whether the treatment is needed, a consultation can be scheduled as a separate appointment. Endodontic treatment is usually completed in one or two visits.
- 8. We provide chair-side TVs and audio options for your relaxation and you are welcome to bring and use your own portable sources of music.
- 9. In office temperature is usually 69°F degrees. A sweater or jacket is recommended for comfort. We provide patients with blankets if needed.
